What care managers do:

  • Assess the level, type of care, hours and frequency needed and develop a care plan
  • Start the care plan and keep it functioning
  • Make sure care is received in a safe and disability friendly environment
  • Mediate family conflicts and other family issues relating to long term care
  • Advocate for the care recipient and empower the family to become advocates
  • Manage care of a loved one for out-of-town families
  • Conduct ongoing assessments to monitor and implement changes in care

  • Coordinate the efforts of key support persons and community services

  • Monitor the care of a family member in a nursing home or in assisted living
  • Health coaching and monitoring compliance with prescribed medications/treatments
  • Create a crisis response plan and evacuation/disaster planning
  • Coordinate and attend medical appointments to facilitate communication
    with medical providers
  • Assist families in positive decision making
  • Develop long range plans for older loved ones not now needing care
